Your debit card is restricted to eligible items at participating retailers and merchants, which can be found online at HorizonExtraBenefits.com or by calling our Horizon EXTRA Benefits Card Member Services line Monday to Friday, 8 a.m. to 8 p.m. Eastern Time (ET), at 1-800-480-6598 (TTY 711).
Over-The-Counter (OTC) Catalog Benefit
You qualify for up to $1,080/year to spend in our OTC Benefit Catalog. That’s $270 every 3 months. Spend it on items from our catalog, like toothpaste, vitamins, denture cleaner and much more. Shop online, by telephone or via mail order.

OTC Benefit + Grocery (retail, online and in-store)
You qualify for up to $1,600/year for Over the-Counter (OTC) and Grocery* purchases. That’s $400 every 3 months. Spend it on select approved benefit items at participating retailers. Shop online or in-store.

- For a full list of participating retailers and eligible items like aspirin and pain relievers, vitamins and minerals, fruits and vegetables, and whole eggs and egg substitutes, visit HorizonExtraBenefits.com.
*Some members may also be eligible to use this benefit to purchase approved grocery items like fruits and vegetables, whole eggs and egg substitutes. To be eligible for the grocery benefit, members must participate in the Care Management program and have any of the following qualifying chronic illnesses: cardiovascular disorders, chronic heart failure or diabetes. Eligible members who meet the criteria may be automatically enrolled into the Grocery benefit, and will receive a letter informing them of enrollment and the funds being added to their card. If you think you may be eligible and have not received a letter, or if you have any questions, reach out to your Care Manager or call Member Services to find out if you qualify. Once you become eligible, you will be able to use your Horizon EXTRA Benefits Card to buy select healthy foods at the start of the next quarter.
Utility Benefit
Eligible* Horizon NJ TotalCare (HMO D-SNP) members can receive an extra $300 per year to help pay for utilities. Utilities are the basic services for your home or apartment needs, like water, electric and gas. (Gasoline is not included.)
To make payments using your Horizon EXTRA Benefits Card, all you need to do is:
- Call your utility provider and use your Horizon EXTRA Benefits Card as form of payment.
- Enter the Horizon EXTRA Benefits Card number on your bill or online.
If your service provider doesn't accept the card, call us at 1-800-480-6598 (TTY 711) or visit HorizonExtraBenefits.com. If your benefit doesn't cover the full amount, contact your service provider to submit an alternate payment option. Rewards & Incentives
As our valued member, your health is important to us. With Horizon Healthy Journey Rewards, you can earn rewards while taking care of your health. Earn up to $235 per year in rewards for completing a routine health survey and these health screenings:
- Annual wellness visit - $25

- Health Risk Assessment - $10
To receive your rewards, visit HorizonExtraBenefits.com to fill out and submit a Health Screening Attestation form. You can also download the form and mail it in. Once confirmed, your rewards will be automatically loaded onto your Horizon EXTRA Benefits Card.
